Referrals by other service providers (e.g., family physician, psychiatrists, psychologists, therapists, etc.), including lawyers, employer organizations, and health care agencies are also welcomed.
Referrals can be faxed to (905) 829-1506 or made by phone at (905) 616-1719. Please note, that unless you have permission from your client to communicate electronically about them over the internet, please do not send referrals by email.
